Inspect that wire (It may have evidence of probing).
This wire may not be connecting with the pin in the controller.
You say you made a breakout adapter for this plug? (nice!).
Is this adapter using nice/tight pins/sockets?
Can you verify Pin13 going low (B- ) with handbrake down? (assuming relay plugged back into socket). Also P13 goes high when Handbrake up.
In case anybody was following this topic this morning, we did not end it on purpose. The forum bots cut Coastal off from further posting (since he is a new guy). He and I took it private until he has full member access.
So far, we are focusing on several bad connections related to the 23p connector.
The Brake light is now OFF
Foot Pedal overvolt appears to be cleared
Now chasing down a few other issues.

5v ref is very low. T4 controller uses pot type throttle and wot wiper should go over 3v. This won’t be possible with 3v ref. Output will always be .5v lower than ref. Not your issue with not running, but will limit performance. My pedals are tested to be over 3v output.
The .1v output, however, will not allow car to move.

We don’t know what that is, but I would love to see one. Even more so that it is overloading pin 9.
Building pot boxes is a challenge as the ref is internally current limited to a very small value.
Pedals are not rebuildable to the best of my knowledge. Bob sends them out and claims no idea what is done to them. Only that they work with no complaints. It’s possible that the internals are swapped to hall effect, which would explain the overloaded ref supply.
Now I’m more interested in you pedal than I am at getting you running.

As is the op. I made breakout boxes for 23p connector for a while. Included programming port and switches to use Sentry monitor. It had traces for status leads, but never went beyond breakout capability.
Ok, trying to see if I got permissions again. Love this group, I am all about it.
As for the pedal I have here, I have complaints but that’s for a different day I suppose. Where we ended up on the day is: several pins in the 23 pin were throwing some gremlins out there. I pulled each pin, adjusted the prongs to ensure solid contact, reseated, and swapped out the red faceplate to a new one with both tabs in tact. Brake light finally working as it should.
Sitting at an error code of 08 on the dash which leads back to the pedal. I have a pedal getting to me tomorrow to try (i had ordered it prior to these conversations). Its from an 02 blazer- found a thread on here that led me down that rabbit hole and figured it was worth a shot. I will let you all know what it looks like once it arrives.
